Does Humana Cover Drug and Alcohol Detox and Rehab?

Humana sells Medicare Advantage, Medicaid and other plans, and each kind follows different rules for detox and rehab coverage. This guide covers what decides coverage, what you may pay, which services need approval first, and how to find a detox center in your plan’s network.
Key Points About Humana Rehab Coverage
- Whether Humana covers detox or rehab depends on which kind of plan you have and that plan’s own documents.
- Humana’s plans are mostly Medicare Advantage and Medicaid, and each kind follows different coverage rules.
- Emergency and urgently needed care does not require prior authorization on Humana’s Medicare Advantage plans.
- Some services need approval before a stay, and what you pay comes out of your plan’s documents.
- You can check your benefits through Humana’s member website, your plan’s app, or the number on your ID card.
- A denied claim can be appealed. There are different routes and deadlines for Medicare Advantage and Medicaid plans.
Does Humana Cover Detox?
Whether Humana rehab coverage pays for detox depends on the kind of Humana plan you have and that plan’s own rules. Humana’s substance use disorders page says Humana plans with behavioral health benefits may include coverage for substance use disorder treatment.
Medicare Advantage plans start from a federal floor. Medicare.gov’s booklet Understanding Medicare Advantage Plans says these plans must cover all medically necessary services that Original Medicare covers.1 When the Centers for Medicare & Medicaid Services guidance for a service, or an indication for a service, is not fully established then Humana develops its own Medicare Advantage coverage policies.2 Those rules establish what your plan pays for a service like medical detox.
Medicaid follows a different structure. Humana’s 2025 Form 10-K describes Medicaid as a program funded by both the federal and state governments and administered by states. Your state program sets the rules for a Humana Medicaid plan.
Emergency care has its own rules, and they split by plan type. On Medicare Advantage, the Medicare Advantage regulations at 42 CFR 422.113 make an MA organization financially responsible for emergency and urgently needed services regardless of whether there is prior authorization for the services.3 Humana’s Medicare Advantage and D-SNP prior authorization and notification list says urgent/emergent services do not require referrals, prior authorization or notification.4 On Medicaid managed care plans, federal Medicaid regulations require the enrollee handbook to state that prior authorization is not required for emergency services.5
If you or someone near you is in immediate harm or danger then call 911. If you need immediate help from a caring counselor then you can call, text or chat with the 988 Suicide & Crisis Lifeline.6 A National Helpline from the Substance Abuse and Mental Health Services Administration is at 1-800-662-HELP (4357).7
Which Humana Plan You Have and Why It Matters
Which set of rules applies to you comes down to the kind of Humana plan you’re on, and Humana runs more than one kind. Humana’s 2025 Form 10-K (the 2025 10-K) says that at December 31, 2025, Humana provided health insurance coverage to approximately 5.2 million individual Medicare Advantage members.8 The rules for those plans don’t come from the same place as the rules for Medicaid or an employer plan.
Medicare Advantage is one kind. As part of its individual Medicare Advantage products, the 2025 10-K says Humana also offers Dual-Eligible Special Needs Plans (D-SNP). The same filing says Humana offers products that enable employers that provide post-retirement health care benefits to replace Medicare wrap or Medicare supplement products with Medicare Advantage or stand-alone PDPs. Those are group arrangements, tied to the employer.
Medicaid is another. The 2025 10-K says Humana has contracts in multiple states to serve Medicaid-eligible members, including Florida, Kentucky, Illinois, Indiana, Louisiana, Ohio, Oklahoma, South Carolina, Virginia and Wisconsin. Kentucky’s Department for Medicaid Services says it uses managed care organizations to provide coverage for most Kentucky Medicaid members.9 That’s Kentucky’s setup. Don’t assume your state’s matches.
Employer coverage is the part that’s changed. Humana’s 2024 Form 10-K says that in February 2023, Humana announced its planned exit from the Employer Group Commercial Medical Products business, which includes all fully insured, self-funded and Federal Employee Health Benefit medical plans. That filing, dated February 20, 2025, said Humana and its subsidiaries anticipate the exit of this line of business to be finalized in the first half of 2025.10
TRICARE is its own program: the contractor for the East Region is Humana Military per TRICARE.11
What Humana Covers by Level of Care
For Medicare Advantage and D-SNP plans, one document sorts the levels. Humana’s Medicare Advantage and D-SNP prior authorization and notification list (the Medicare Advantage list), effective July 1, 2026. It names mental health and substance use treatment under inpatient admissions, including any treatment in a residential setting. It also names partial hospitalization under behavioral health services, and it says certain services may not be covered under the member’s plan.
The Medicare Advantage list names no detox, withdrawal management or intensive outpatient service. A person weighing inpatient detox or outpatient detox won’t find either one named there.
Intensive outpatient care is on a different list, and it’s Medicare’s, not Humana’s. Medicare.gov lists intensive outpatient program services under Medicare Part B. This includes care you’d receive at a hospital, community mental health center, Federally Qualified Health Center, Rural Health Clinic or Opioid Treatment Program.12
Kentucky Medicaid runs on its own documents. Humana’s Kentucky Medicaid behavioral health services page lists substance use services (including residential services) for all members, regardless of age.13 Humana’s Kentucky Medicaid prior authorization and notification list (the Kentucky list), effective March 1, 2026 includes all residential treatment services in its residential treatment row.14 That list says it does not apply to Humana Medicare or other state Medicaid plans. Kentucky’s rules stay in Kentucky.
| Level of care | Humana Medicare Advantage and D-SNP list (effective July 1, 2026) | What Humana’s list and Medicare.gov say |
|---|---|---|
| Inpatient mental health and substance use treatment | On the list | Listed under inpatient admissions |
| Residential treatment | On the list | Inpatient admissions include any treatment in a residential setting |
| Partial hospitalization (PHP) | On the list | Listed under behavioral health services |
| Intensive outpatient (IOP) | Not on the list | Medicare Part B lists intensive outpatient program services |
| Outpatient services | Not on the list | Medicare Part B lists outpatient services you might get as part of substance use disorder treatment |
Who Manages Humana’s Behavioral Health Benefits
The name is Humana Behavioral Health Network. Humana’s 2026 provider manual lists Health Value Management Inc., d/b/a Humana Behavioral Health Network (HBHN), as a wholly owned subsidiary of Humana Inc.15
HBHN is Humana’s behavioral health network. The manual states that it includes more than 60,000 providers and 1,800 facilities across all 50 states and the District of Columbia. Behavioral health is the umbrella term, and it covers a lot of ground, from addiction treatment to therapy for co-occurring mental health conditions.
The manual doesn’t say which Humana plans use HBHN, or how it handles any request.
What Detox Costs With Humana
What you pay for detox with Humana depends on the kind of plan you have and that plan’s own costs. Part C premiums, deductibles and copays vary by plan, and all Medicare Advantage plans offer an annual maximum out-of-pocket limit, according to Humana’s page on Medicare out-of-pocket costs.16 Medicaid runs on different rules: states have the option to charge premiums and to establish out of pocket spending (cost sharing) requirements, per Medicaid.gov.17
The math starts with what detox costs, and the plan decides what comes off that bill. On a Medicare Advantage plan, your share has a ceiling. Plans have a yearly limit on what you pay for covered Part A and Part B services (which may include different limits for in-network and out-of-network services), per Medicare.gov’s booklet Understanding Medicare Advantage Plans. That limit is the most you’d pay in a year for those covered services, whether the bill arrives as a deductible or a copayment.
Medicaid has its own ceiling. Premiums and cost sharing for everyone in the household may not exceed an aggregate limit of 5 percent of the family’s income, under federal Medicaid regulations. The state agency decides whether the limit applies quarterly or monthly.18
Prior Authorization and Medical Necessity
Whether a detox admission needs approval first varies by plan. Humana’s Medicare Advantage and D-SNP prior authorization and notification list says the full list of prior authorization requirements applies to patients with Humana MA HMO and HMO point-of-service (HMO POS) coverage, and the list says it also applies to patients with Humana MA PPO coverage. Prior authorization is not required for MA PFFS plans.
There’s one break for people who just joined. To prevent disruption of care, the list says Humana does not require prior authorization for basic Medicare benefits during the first 90 days of a new member’s enrollment for active courses of treatment that started prior to enrollment. Every condition in that sentence is part of the rule: basic Medicare benefits, an active course of treatment, and a start date before enrollment.
Once a stay starts, the review doesn’t stop. Humana’s 2026 provider manual says concurrent review is the process that determines coverage during the inpatient stay, including, but not limited to, acute inpatient facility, skilled nursing facility, long-term acute care hospital, inpatient rehabilitation facility and behavioral health partial hospital/residential treatment facilities. Both the approval before admission and the reviews during a stay turn on medical necessity, meaning the plan’s judgment about whether inpatient detox is needed.
Federal rules set the clock for these decisions. Medicare Advantage, Medicaid and CHIP plans must send prior authorization decisions within 72 hours for expedited, or urgent, requests and within seven calendar days for standard ones, according to the Centers for Medicare & Medicaid Services (CMS).19 Its Interoperability and Prior Authorization Final Rule, CMS-0057-F, carries a compliance date of January 1, 2026, and CMS notes the rule does not apply to prior authorization decisions for drugs.
Medications for Addiction Treatment
Medicare Part B covers methadone, buprenorphine, and naltrexone when you get them in a doctor’s office or through an Opioid Treatment Program, according to Medicare.gov.20 Humana’s 2026 formulary for HumanaChoice (PPO) and other plans, updated September 2, 2026, lists buprenorphine-naloxone sublingual tablets on Tier 2.21
The same formulary lists naltrexone tablets, also on Tier 2. It doesn’t list Suboxone. It’s one Humana formulary among several, and it says Humana may add or remove drugs on the formulary during the year.
Medicare drug coverage (Part D) may cover buprenorphine and naltrexone, Medicare.gov says.
Medicaid runs on different rules. Section 201 of the Consolidated Appropriations Act, 2024 made permanent the mandatory Medicaid benefit for medications for opioid use disorder, per the Centers for Medicare & Medicaid Services (CMS).22 That benefit requires states to cover all drugs approved to treat opioid use disorders, including methadone, CMS says.
Whether a drug like methadone needs prior authorization depends on the plan.
Detox Centers and Rehabs That Accept Humana
Start with Humana’s own search. Humana’s Find Care flyer says you can use its Find Care search tool to find in-network doctors, dentists, pharmacies, and more.23
Two ways in. Sign in to your secure MyHumana account to conduct a search, or search as a guest by entering your location, the flyer says. Once you’re enrolled with Humana, the flyer notes, the MyHumana mobile app can find a provider near you.
The tool won’t tell you everything about a program. When you find a detox center that looks right, whether it takes your plan is the first question, not the only one: choosing a detox center comes down to more than a network result.
How to Check Your Humana Benefits
Sign in to MyHumana and open your Plan benefits. That’s where your benefit information is, according to Humana’s member support page. To confirm that a service will be covered, the same page says to refer to your Evidence of Coverage or call Customer Service at the number on the back of your Humana ID card.24
You’ll want the card in hand. Within 10 business days of enrollment, you can view, print, email or order it in the ID Card Center or MyHumana Mobile app, per the member support page. It’s the piece everything else hangs on.
Go in knowing one limit. The member support page says its communication does not guarantee benefits, and it does not indicate all services received will be covered by your plan. That’s Humana’s own limit, in its own words. The Evidence of Coverage is what settles the question.
The same habit holds beyond Humana: read the plan’s own documents before using insurance for detox, whatever the carrier.
There’s one more number on the back of the card. Humana’s substance use disorders page points members to the Humana Behavioral Health Hotline number found on the back of your Humana member ID card.

If Humana Denies Coverage
The route depends on which plan you have. On a Medicare Advantage plan, Humana’s grievances, appeals and exceptions page gives you 65 days from the date of the denial notice to appeal, and Medicare.gov’s page on appeals in Medicare health plans says there are 5 levels of appeals.25 On Humana’s Kentucky Medicaid plan, you must file an appeal within 60 days of the adverse benefit determination, and you can ask for a state fair hearing after you complete the Humana appeal process, according to Humana’s Kentucky Medicaid grievance and appeal page.
For Medicare Advantage, the 65 days run from the date on Humana’s Notice of Denial of Medical Coverage (or Payment). There’s a faster route, but it’s narrow. Humana’s appeals page says expedited appeals are only appropriate for services that haven’t yet been rendered, and only when you and your provider believe waiting for a standard decision could seriously harm your life, health or ability to regain maximum function.
Medicare’s 5 levels start with your plan reconsidering its own decision. Medicare.gov gives the plan 30 days to decide a pre-service appeal at level 1. A fast appeal moves quicker: the plan must decide within 72 hours if it determines, or your doctor tells your plan, that waiting for a standard decision may seriously jeopardize your life, health, or ability to regain maximum function. If your plan upholds their denial at level 1, they’ll automatically forward their reconsideration decision to an Independent Review Entity (IRE) to start a level 2 appeal, according to Medicare.gov.26
Kentucky Medicaid runs on its own clocks. Humana’s Kentucky page says an appeal may take up to 30 days to process.27 After you complete the Humana appeal process, you have the right to ask for a state fair hearing from the Department for Medicaid Services, but you must ask within 120 days from the date on Humana’s appeal decision letter.
Those deadlines are Kentucky’s. They don’t apply to another state’s Medicaid plan or to Medicare Advantage. And two sets of rules you may have read about elsewhere, the Department of Labor’s claims deadlines and HealthCare.gov’s external review, don’t apply to a Medicare Advantage or Medicaid plan.
FAQs About Humana Rehab and Detox Coverage
Your plan and the setting decide it. Humana’s Medicare Advantage and D-SNP prior authorization and notification list (Humana’s prior authorization list) includes mental health and substance use treatment, including any treatment in a residential setting, under inpatient admissions. It also notes that certain services may not be covered under the member’s plan. Medicare.gov lists Part A services for a person admitted to a general or psychiatric hospital as an inpatient.
Humana sets no fixed number of days. Humana’s 2026 provider manual describes concurrent review as the process that determines coverage during an inpatient stay, including behavioral health partial hospital/residential treatment facilities. Each admission is reviewed for medical necessity and compliance with contractual requirements, the manual says. To extend a stay past the days first authorized, the facility must submit a subsequent request for continued stay along with the most recent medical records.
A Humana Medicare Advantage plan must cover all medically necessary services that Original Medicare covers, according to Medicare.gov’s booklet Understanding Medicare Advantage Plans. Medicare.gov’s Part B list includes outpatient services you might get as part of substance use disorder treatment, and intensive outpatient program services at a hospital, community mental health center or Opioid Treatment Program. Whether your plan pays for a specific service depends on your plan’s terms.
The medication and your plan decide it. Medicare Part B covers methadone, buprenorphine, and naltrexone when you get them in a doctor’s office or through an Opioid Treatment Program, per Medicare.gov, and Part D may cover buprenorphine and naltrexone. Humana’s 2026 formulary for HumanaChoice (PPO) and other plans lists buprenorphine-naloxone sublingual tablets on Tier 2.
Your plan type decides it. Medicare.gov’s HMO page says that in most cases, you have to get a referral to use a specialist in an HMO plan.28 Humana’s prior authorization list applies to patients with Humana MA HMO and HMO point-of-service (HMO POS) coverage. Urgent and emergent services do not require referrals, prior authorization or notification, the list notes.
HMO or PPO decides it. Humana’s 2025 Form 10-K states that except in emergency situations or as specified by the plan, most HMO plans provide no out-of-network benefits. Medicare.gov’s PPO page says you can use out-of-network providers if they’re participating in Medicare or accept assignment, but you’ll usually pay more, and tells you to contact your PPO Plan first.29 Emergency services are covered regardless of whether the provider that furnishes the services has a contract, under federal Medicaid regulations.30
