About Center for Comprehensive Health Practice
The Center for Comprehensive Health Practice (CCHP) is a community-based integrated care provider serving adults, adolescents, children and their families throughout East Harlem, New York. They are located on East 110th Street in New York City, less than a mile from Central Park, the Studio Museum in Harlem and El Museo del Barrio. It’s a vibrant and supportive outpatient setting enriched by the neighborhood’s history and creativity.
Here, you can receive targeted support to navigate your substance use and mental health challenges alongside primary medical issues via the integrated care model. With a focus on improving the overall well-being of individuals and families across East Harlem, CCHP creates an inclusive environment that welcomes everyone regardless of their background, age and sexual orientation. They support LGBTQ+ members and individuals living with HIV/AIDS and offer dedicated treatment options for pregnant and postpartum women.
To ensure affordable access to care, the agency accepts Medicaid, Medicare and CHIP. They also offer a sliding fee scale based on income, so no one is denied services due to an inability to pay.
Medication-Supported Opioid Recovery
MAT at CCHP involves personalized methadone prescriptions to ease painful opioid withdrawal and reduce cravings so you approach recovery with greater stability and confidence. Buprenorphine-based MAT is also available with home-based induction when appropriate. In both cases, your care plan will include regular outpatient therapy to help you develop coping skills and relapse prevention strategies that support long-term abstinence while staying grounded.

CCHP’s dedication to supporting each person’s unique needs is commendable. They offer trauma-informed and compassionate therapeutic support to individuals and families geared towards maintaining mental stability, improving communication and strengthening relationships. Play therapy is available for children over five experiencing emotional or social challenges.

Outpatient
In outpatient therapy, you’ll attend therapy sessions several times each week while living at home. This is ideal if you have a strong support system and a lower risk of relapse. Outpatient treatment offers flexibility to maintain work, school or family obligations.

Dual diagnosis programs address substance use disorders and co-occurring mental health conditions simultaneously. This integrated approach to care improves the likelihood of long term recovery and stability by addressing the root causes of addiction.

Outpatient Detox
Outpatient detox gives you access to medically supervised withdrawal services while still allowing you to live at home. You’ll attend a clinic for treatment and monitoring. This flexible option is suitable for those with mild to moderate withdrawal symptoms who have strong support systems.

Adult programs address the substance use and life challenges specific to adults. Therapists can deliver sessions in individual, group and family settings. Services often include job support and life skills training in a structured environment.

Alcohol detox programs offer medical support to help individuals withdraw safely from alcohol. Your care team may use medications to ease your symptoms and provide medical monitoring to address complications.

Men's programs address substance use while also considering the social pressures, family roles and mental health concerns that are specific to men. You’ll learn healthy coping mechanisms as you build emotional resilience and develop communication skills.

Opioid detox uses medications to ease severe withdrawal symptoms. It also includes medical supervision to help you manage potential complications. These services allow you to stabilize and begin a recovery plan.

Women's programs offer a safe and supportive space to focus on gender specific issues such as trauma, family roles and mental health conditions. Therapists tailor the sessions to address women's needs and foster empowerment in a healing and nurturing environment.

Young Adult (18 - 25)
Young adult programs are designed for individuals who are transitioning into adulthood. Topics of discussion typically include identity, independence and peer relationships. Providers may also offer life skills training and career support.
