About Sage Prairie
Sage Prairie provides evidence-based and personalized opioid recovery services for adults in Burnsville and the greater Dakota County area of Minnesota. They assist with alcoholism, marijuana use disorder and addictions to stimulants such as cocaine and crystal meth. They can even help you overcome co-occurring psychiatric depression. Sage Prairie is in southern Burnsville, south of Crystal Lake and west of the Orchard Gardens Railway Station
The truly impressive thing about Sage Prairie is that it gives you the chance to take an active role in your recovery and heal at your own pace and terms. You’re equipped with resources and an experienced team who works to create a welcoming environment where everyone feels respected.
This ensures support is extended to individuals from diverse backgrounds, including LGBTQ+ members, those living with HIV/AIDS and people in legal trouble. Pregnant and postpartum women, along with veterans and active-duty military personnel, also benefit from the care offered. The clinic accepts Medicaid to reduce financial barriers.
Unified Approach to Long-Term Wellness
Sage Prairie promotes a strategy that includes medication-assisted treatment (MAT), psychiatric care, peer support and case management. Prescribed FDA-approved Suboxone or Vivitrol can ease painful withdrawal symptoms and reduce cravings.
You’ll attend therapy to develop healthier ways to cope and prevent relapse. Counseling is accessible either via outpatient care or a structured intensive outpatient program for long-term abstinence.
At the same time, trained psychiatrists provide medication management and psychotherapy to address co-occurring mental health conditions and help you stabilize emotionally. You’ll have peer specialists by your side throughout your journey offering insight. Through case management, you’ll receive tailored assistance to navigate challenges like housing, employment, transportation and access to medical care.
Aftercare support may include self-help or peer support groups and connections to community resources to help maintain your progress and prevent relapse.

Levels of Care
-
Outpatient

-
Aftercare
Aftercare programs provide ongoing support after you complete a rehab program. They may include several components to help you maintain sobriety including therapy, community support groups and relapse prevention strategies. This gives you a network of resources as you reintegrate into your daily life.
-
PHP
Partial hospitalization programs provide comprehensive treatment in a structured setting during the day but allow you to return home at night. These programs offer a balance of inpatient and outpatient rehab and provide intensive support without full time residency.
-
Sober Living
Sober living homes provide a supportive and substance free environment for you to live in as you overcome your addiction. Residents must follow house rules and support each other's recovery journeys. Sober living fosters accountability and stability during this critical phase of recovery.
-
Intervention
An intervention is a structured and professionally guided conversation with an individual who is struggling with addiction. During the conversation, family and friends will encourage you to seek treatment. This is often a pivotal step for those resistant to getting help.
Detox Service Setting
-
Outpatient Detox
Outpatient detox gives you access to medically supervised withdrawal services while still allowing you to live at home. You’ll attend a clinic for treatment and monitoring. This flexible option is suitable for those with mild to moderate withdrawal symptoms who have strong support systems.
-
MAT
Medication assisted treatment combines medication and counseling to manage withdrawal and reduce cravings for opioid and alcohol addiction. Medications may include methadone, buprenorphine or naltrexone. MAT is tailored to your needs so you can actively participate in your treatment journey.
