About Project Transition

Project Transition is located in Reading, Pennsylvania, and offers community based treatment for individuals with serious mental illness, co-occurring substance use disorder (SUD) or developmental/intellectual disabilities. 

The programs include residential services and a system of support (SOS) program. The residential program includes outpatient day programs. The program does not include detox, but the staff can refer clients for detox.

The organization accepts private insurance, Medicare, Medicaid and some state financed or funded insurance plans. The main facility is about six miles from downtown Reading, which features a walkable urban center.

Community Living for Recovery

Individuals live in apartments and share furnished rooms with other program members. Although program members live together, most of your neighbors won’t be affiliated with the program.

This is to provide members with responsibility and normalcy while reducing stigma and facilitating integration.

WRAP and Systems of Support

Every client has a wellness recovery action plan (WRAP). WRAP includes vocational and educational services to assist clients in achieving independence. 

Systems of support (SOS) assist clients in achieving and maintaining a better quality of life within their community. With help, clients build an individual support system of friends, family and health providers. Services include crisis intervention and prevention.
