About OPI Houston (Formerly Fulshear Treatment to Transition)
OPI Houston, formerly Fulshear Treatment to Transition, is a residential treatment and transitional living program for young adults ages 17.5 to 28 near Needville, Texas. The program treats depression, anxiety, trauma, PTSD, mood and personality disorders, attachment issues and addiction through a blend of evidence-based therapies and life skills development. OPI Houston’s model moves clients from a therapeutic ranch to city-based apartment living, supporting a structured bridge from crisis stabilization to independent adulthood.
Residential Treatment and Transitional Living for Young Adults
OPI Houston specializes in guiding young adults through a step-down continuum that starts with 24/7 residential care at The Ranch and progresses to PHP and IOP levels with apartment housing. This ranch-to-city model provides therapy, community integration and real-world skills practice without changing providers. Clients can also return to the ranch for a reset if they struggle during the transition phase, a “fail-forward” approach that normalizes setbacks.
Programming includes individual, family, and group therapy using common modalites, as well as Accelerated Resolution Therapy. Sessions target underlying trauma and attachment patterns while building executive function and daily living skills.
Attachment Focused Care and Equine Therapy
The program is grounded in the Integrated Adult Attachment Model, emphasizing relational healing over behavior correction alone. EAGALA-certified, equine-assisted psychotherapy is integrated into residential treatment, offering experiential work in a therapeutic ranch setting with no riding required.
Payments with OPI Houston
OPI Houston serves young adults at its ranch near Needville and city apartments in the Houston area, with levels ranging from residential to IOP with housing. The program is out-of-network with most plans and accepts private insurance and self-pay, with an in-house team to verify benefits.
Levels of Care
-
Outpatient
In outpatient therapy, you’ll attend therapy sessions several times each week while living at home. This is ideal if you have a strong support system and a lower risk of relapse. Outpatient treatment offers flexibility to maintain work, school or family obligations.
-
Dual Diagnosis
Dual diagnosis programs address substance use disorders and co-occurring mental health conditions simultaneously. This integrated approach to care improves the likelihood of long term recovery and stability by addressing the root causes of addiction.
-
Aftercare
Aftercare programs provide ongoing support after you complete a rehab program. They may include several components to help you maintain sobriety including therapy, community support groups and relapse prevention strategies. This gives you a network of resources as you reintegrate into your daily life.
-
Sober Living
Sober living homes provide a supportive and substance free environment for you to live in as you overcome your addiction. Residents must follow house rules and support each other's recovery journeys. Sober living fosters accountability and stability during this critical phase of recovery.
-
Intervention
An intervention is a structured and professionally guided conversation with an individual who is struggling with addiction. During the conversation, family and friends will encourage you to seek treatment. This is often a pivotal step for those resistant to getting help.
Detox Service Setting
-
Hospital Detox
Hospital detoxification allows you to withdraw from drugs or alcohol in a hospital setting. It’s ideal for people with severe withdrawal symptoms or complex medical needs. You’ll receive constant monitoring, emergency care if needed and FDA approved prescription medications.
-
Outpatient Detox
Outpatient detox gives you access to medically supervised withdrawal services while still allowing you to live at home. You’ll attend a clinic for treatment and monitoring. This flexible option is suitable for those with mild to moderate withdrawal symptoms who have strong support systems.
-
MAT
Medication assisted treatment combines medication and counseling to manage withdrawal and reduce cravings for opioid and alcohol addiction. Medications may include methadone, buprenorphine or naltrexone. MAT is tailored to your needs so you can actively participate in your treatment journey.
Programs
-
Adult (18+)
Adult programs address the substance use and life challenges specific to adults. Therapists can deliver sessions in individual, group and family settings. Services often include job support and life skills training in a structured environment.
-
Women

-
Cognitive Behavioral Therapy
Cognitive behavioral therapy focuses on changing harmful thought patterns and behaviors associated with addiction. You’ll learn healthier coping mechanisms by identifying and replacing negative thoughts. This improves your emotional resilience and decreases your relapse potential.
-
EMDR Therapy
EMDR stands for eye movement desensitization and reprocessing therapy. It helps you process traumatic memories that may underlie addiction. You’ll learn how to reduce emotional distress and begin healing from past events. This makes it effective for trauma related addiction causes.
-
Animal Therapy
Animal therapy uses interactions with animals such as dogs or horses as part of treatment. You’ll learn to develop trust, lower your anxiety levels and regulate your emotions. This provides comfort and a sense of companionship during the recovery journey.
