About Discovery Transition Outpatient

Discovery Transition Outpatient is an addiction treatment center in Van Nuys, California that serves adults throughout the San Fernando Valley. The center offers intensive outpatient and standard outpatient programs, plus aftercare support for individuals transitioning from residential care or beginning their recovery journey.

Addiction Treatment Programs

Discovery Transition Outpatient provides an Intensive Outpatient Program that allows clients to live at home or in sober living while attending structured group sessions and individual counseling.

The center also offers a Standard Outpatient Program for ongoing recovery maintenance and aftercare services for those stepping down from inpatient treatment. Medical and mental health assessments, personalized care planning, and comprehensive case management support each client’s journey.

The program serves adult men and women struggling with alcoholism and drug addiction, including those with co-occurring mental health conditions requiring dual diagnosis care. Trauma informed treatment is available for clients whose addiction is connected to underlying trauma.

Treatment Approaches and Detox Referral

Treatment includes Cognitive Behavioral Therapy, Neurofeedback, intensive individual and group counseling, family therapy, and 12-Step facilitation. Life skills training addresses emotional regulation, stress management, coping strategies, and relapse prevention, while recovery coaches guide clients through early sobriety challenges.

Although the center only offers medication management for psychological disorders they can refer clients to a local detox treatment facility.
