Downtown Family Health Center at Connections provides substance use treatment in San Diego, California. The team offers numerous services that include integrated behavioral health care. Co-located with PATH Connections Housing, the clinic specializes in serving people experiencing homelessness alongside community members seeking medical and mental health support in one convenient location.
Medications to Reduce Withdrawal
The center provides outpatient substance use disorder care, including medication-assisted treatment with buprenorphine and naltrexone for opioid use disorder, plus methadone services. These medicines help lessen symptoms and cravings so guests can remain comfortable and safe during treatment.
Individual counseling, case management, relapse prevention, cognitive behavioral therapy, motivational interviewing, and telehealth options support long-term recovery. Case managers can provide connections to outside agencies and community resources for ongoing recovery assistance.
Inclusive Programming
Services are open to adults of all ages, including transitional age young adults, pregnant and postpartum women, seniors, LGBTQ clients, veterans and individuals involved in the criminal justice system. The clinic works with clients who have co-occurring mental health conditions, HIV or hepatitis, or a history of trauma or domestic violence.

Outpatient detox gives you access to medically supervised withdrawal services while still allowing you to live at home. You’ll attend a clinic for treatment and monitoring. This flexible option is suitable for those with mild to moderate withdrawal symptoms who have strong support systems.

MAT
Medication assisted treatment combines medication and counseling to manage withdrawal and reduce cravings for opioid and alcohol addiction. Medications may include methadone, buprenorphine or naltrexone. MAT is tailored to your needs so you can actively participate in your treatment journey.
